OR
$60 for 4000 ccpoints (min $200 deposited and played in past 4 weeks)
OR
$125 for 6000 ccpoints (min $500 deposited and played in past 4 weeks)
By "past 4 weeks" is the 4 weeks prior to you making your claim.
